Accountability process

How corrections work

Anyone can challenge the record — but every correction must include a suggested fix, a source, and a verified account. A reviewer accepts it (applying the change and chaining a tamper-evident revision) or rejects it, in the open. Original records are never erased; this page is the public log of what changed, who proposed it, and the outcome.
