Trump's first term produced a partial defund: an HHS final rule published March 4, 2019 prohibited Title X grantees from referring patients for abortion, and Planned Parenthood withdrew from the Title X program in August 2019 rather than comply, while its federal Medicaid reimbursements continued. In his second term, the reconciliation law signed July 4, 2025 barred Medicaid payments to major abortion providers for one year, and a federal appeals court allowed that cut to take effect in September 2025 while litigation continues. Because the Medicaid prohibition is time-limited to one year and Planned Parenthood retained most federal funding through his first term, the promised defunding was achieved in substantial part but not completely or permanently.
